Patient Rights and Responsibilities
Stevenson Memorial Hospital endeavours to provide quality patient care, delivered by qualified health care professionals. The hospital adopts the following statement of Patient Rights and Responsibilities.
Patient Rights Patients have a right to:
- Be treated with dignity and respect
- Care that is reasonably available and provided by competent professionals
- Privacy and confidentiality of their health information
- Be given relevant information to foster informed decisions concerning their care.
- Access their clinical record, in accordance with legislation
- Make decisions related to their care
- Be provided with investigation and treatment which offers a reasonable expectation of benefit
- Be aware of their rights and responsibilities
Patient Responsibilities Patients are responsible for:
- Participating in their treatment and care
- Complying with hospital rules, for example, visiting hours, no smoking, no cellular phones
- Respecting the rights of others

For Your Admission
The following information will assist you in preparing you for admission to Stevenson Memorial Hospital.
Parking Parking is available at a flat fee of $5 daily. Tickets are obtained at machines in the parking lots. A change machine is located at the entrance of the lower level. Due to space restictions, we regret we are not able to have a machine on the main level at this time. Please park in designated areas only and be aware of heliport restrictions. Vehicles must be moved when the Medevac helicopter is flying in.
Be sure to bring the following:
- Your own Ontario Health Insurance (OHIP) Card
- Details of any additional medical insurance coverage
- A list of medication, including prescription and over the counter products which you are currently taking or have taken recently
You will be asked to provide information on:
- Your current home address, with postal code and telephone number
- Whether you want allow visitors, phone calls and visits from your spiritual care giver.
- The name of a spouse, partner or next of kin and information on how they can be contacted
- Any allergies you might have to food, medication or other substances
Personal items you will require in hospital:
- Dressing gown and slippers
- Tissues
- Soap and shampoo
- Lotion
- Sanitary items
- Dentures
- Razor charged or battery operated
- Eye glasses
- Toothbrush, toothpaste, comb and deodorant
- Reading material
All personal items brought into the hospital should be labeled with your name and phone number. Stevenson Memorial will not be responsible for money or valuables brought into the hospital.
Charges not covered by OHIP Ontario residents have almost all their hospital care covered by the Ontario Ministry of Health and Long Term Care. There are however, additional service charges a patient is responsible for paying, such as:
- Telephone calls
- Crutches or other orthopaedic aids
- Television rentals
- Inspirometer
- Semi-private accommodation
Non-residents of Canada must pay room rates as well as the above items. For information regarding hospital charges, please contact Patient Accounts at extension 3206.

The following information may be of assistance during a patient's stay.
Meals Breakfast is served at 8:15 a.m., while lunch is served at noon and supper at 5:00 p.m.
Meals are served in the patient's room. A cafeteria on the lower floor provides service for visitors and the Auxiliary operates the Coffee Corner on the main floor. Vending machines are also available.
Telephones All in-coming calls go through the Switchboard at (705) 435-6281 or (705) 435-3377.
Telephones are provided in patient's room, at a daily rate. If you wish to have this service, please request it on admission or contact the Switchboard. Calls will be directed to rooms by name or extension number.
- To place a local call
Dial 9 for an outside line and then dial the number you want to reach.
- To place a long-distance call
Dial 9, then 0 and the number you are calling. Only credit card and collect calls can be placed.
Public telephones are located in the Emergency Room and on the lower floor by the cafeteria.
Cellular phone use is prohibited in the hospital since their frequency can interfere with the operation of patient care equipment.
Television and Radio A television rental service is available. Requests must be made upon admission at the Switchboard or to the unit nurse. Rental charges are to be paid in advance at the Switchboard.
If you wish to bring your own radio, a small battery operated one is safe to use. To avoid distracting other patients, we ask that you wear earphones. Electric radios must be checked by the Maintenance Department before use is permitted. The hospital does not accept any liability for damage or loss.
Mail Mail is delivered to patients' rooms daily. The mailing address at the hospital is: Stevenson Memorial Hospital 200 Fletcher Crescent, P.O. Box 4000 Alliston, Ontario L9R 1W7
Discharge Time The time of discharge is set at 9:30 a.m. Please arrange for your transportation to arrive before that time.
Information If you need further information or have any concerns regarding your hospital visit, please contact the nurse on your unit. Arrangements will be made for you to discuss your concerns with the appropriate personnel.

Spiritual Services The Spiritual Services Department provides support to people of every belief or value system. It offers emotional and spiritual support to patients and their families.
The department provides caregivers who have special listening skills. Members of the Spiritual Care Team also meet religious needs of patients through the celebration of their chouice of rites or sacraments, by praying with people when requested to do so and by providing devotional aids.
Whenever there is a need for spiritual services, please ask staff or the operator to contact the Spiritual Services Department.
Discharge Planning Patients may need the assistance of community support services or institutional care for a period of time. The SMH Discharge Planner will work with patients, their families and members of their health care team, to establish a link with the necessary resources. Patients should discuss their discharge needs with their physicians as early as possible in order to give the Discharge Planner time to help make the appropriate arrangements.
Community Care Access Centre (Home Care) Home Care provides a variety of nursing, homemaking and therapy services for eligible clients. It may be arranged through the family physician, Discharge Planning or directly by a patient or their family. The service, for eligible patients, is funded by the Ontario Ministry of Health. The Mary McGill Community Mental Health Centre Adjacent to the hospital is the Mary McGill Community Mental Health Centre. It is a department of Stevenson Memorial Hospital. Individual, family and marriage counselling and psychiatric consultation services are available. Referrals for adults and children are accepted from doctors, agencies, or the clients themselves. M.O.R.E. (Multiple Organ Retrieval Exchange) Stevenson Memorial Hospital supports organ and tissue donation and participates in education and identification of potential donors.
